Terms of Service

Welcome to the Centruva teaser and waitlist website (the “Site”). The Site is operated by Centruva LLC, a Management Services Organization (“MSO”). Centruva LLC provides administrative, platform, and marketing services, and connects patients to our affiliated professional medical practices. By accessing this Site or joining our waitlist, you agree to these Terms of Service.

1. PRE-LAUNCH DISCLOSURE

AFFILIATED MEDICAL ENTITIES ARE NOT YET OPERATING AS CLINICAL PRACTICES IN ALL JURISDICTIONS. CENTRUVA LLC IS A MANAGEMENT SERVICES ORGANIZATION AND DOES NOT PROVIDE MEDICAL SERVICES. THIS SITE IS FOR INFORMATIONAL AND WAITLIST PURPOSES ONLY.

By joining the waitlist, you acknowledge that:

  • We are in a pre-launch phase. Centruva LLC does not provide medical services, evaluations, or prescriptions.
  • Joining the waitlist does not guarantee future access to services, clinical approval, or a prescription.
  • Clinical services, when launched, will be provided exclusively by our affiliated professional medical practices, and will be subject to a separate patient agreement, medical screening, and physician review.

2. NO MEDICAL ADVICE

THE CONTENT ON THIS SITE IS FOR GENERAL INFORMATIONAL PURPOSES ONLY. IT IS NOT MEDICAL ADVICE AND IS NOT A SUBSTITUTE FOR PROFESSIONAL MEDICAL ADVICE, DIAGNOSIS, OR TREATMENT.

Always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay in seeking it because of something you have read on this Site.

3. NO PROVIDER-PATIENT RELATIONSHIP

ACCESSING THIS SITE OR JOINING THE WAITLIST DOES NOT ESTABLISH A PHYSICIAN-PATIENT, PROVIDER-PATIENT, OR CLINICAL RELATIONSHIP BETWEEN YOU AND CENTRUVA LLC, ANY AFFILIATED MEDICAL PRACTICE, OR ANY INDIVIDUAL PROVIDER.

A provider-patient relationship is only established after you complete a formal clinical intake process and a physician has reviewed and accepted your case, following our official launch.

4. ELIGIBILITY

This Site is intended for use by residents of the State of Texas who are at least 18 years of age. By using this Site, you represent that you meet these eligibility requirements. Our future clinical services may have additional age or residency restrictions (e.g., ages 21–65).

5. USE OF THE SITE

You agree to use the Site only for lawful purposes. You are prohibited from:

  • Providing false or misleading information, including fake email addresses.
  • Attempting to interfere with the Site’s security or abuse the waitlist system.
  • Using any automated system (bots, crawlers) to access the Site without our permission.

6. LIMITATION OF LIABILITY

TO THE MAXIMUM EXTENT PERMITTED BY LAW, CENTRUVA AND ITS AFFILIATES SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF YOUR USE OF THE SITE OR WAITLIST.

7. CHANGES TO TERMS

We reserve the right to modify these Terms at any time. Any changes will be effective immediately upon posting to the Site. Your continued use of the Site after changes are posted constitutes your acceptance of the new Terms.

8. GOVERNING LAW

These Terms are governed by the laws of the State of Texas, without regard to its conflict of law principles. Any disputes arising from these Terms or the Site shall be resolved in the courts located in Texas.
